Print Insurance Form Icon

The Print Insurance Form    icon on the Transaction screen is used to process an insurance claim or pre-authorization, batch an electronic insurance claim (eClaim), or print an Attending Physician’s Report.

MOGO will know if the claim is a pre-authorization if you select undated treatment, or if it is a claim for payment if you select dated treatment.

The drop down menu next to the Print Insurance Form icon is used for Electronic Eligibility (eEligibility), which is a service provided by Change Healthcare that allows you to check a patient’s insurance coverage/benefits electronically.
